Appointments and Office Practices
Appointments for new and follow-up visits can be made by calling our offices Monday through Friday from 8:30 a.m. to 4:30 p.m. In fairness to other patients, and the doctor, we require at least 24 hours notice to cancel appointments. You may be charged for missed appointments.To provide high quality and efficient care, we must work within an appointment schedule. You should bring your insurance cards and relevant referral forms so our staff may better serve you.If you have printed the new patient forms from our website (www.entpsa.com), please complete them and give them to the receptionist at check-in. Minors must be accompanied by their parent or legal guardian. We want your visit to our office to be as comfortable as possible. If we fall behind schedule due to an emergency or other unforeseen situation, we ask for your understanding. You will receive the same care and attention as those seen before you.
Fees and Payments
We ask that you pay for our services or pay any deductible or co-payments at the time service is rendered. We will also collect any coinsurance and/or deductibles before surgical services are rendered.We bill most insurance carriers for you if proper paperwork is provided to us. We will also bill most secondary insurance companies for you. If an insurance carrier has not paid within 60 days of billing, professional fees are due and payable in full from you. For Medicare patients; we will bill Medicare and your secondary insurance for you.If you have no insurance we would appreciate payment at the time of service. We accept Visa, Master Card, Checks, Cash, and in certain circumstances Care Credit.®.We take great pride in our capability to serve you and our goal is to achieve a mutually satisfactory relationship.
